Background: Rhinophyma is a rare, severe form of rosacea characterized by progressive thickening and nodular deformation of the nasal skin due to sebaceous gland hyperplasia and fibrosis. It predominantly affects older men and can lead to significant cosmetic and functional concerns. Management can involve the use of topical agents, combination of laser therapy and dermabrasion. In regional settings like Northern Queensland, limited access to advanced equipment presents additional challenges, requiring clinicians to adapt their approaches to provide effective care.
Case Report: A 60-year-old male presented with advanced rhinophyma, resulting in marked nasal enlargement and distortion. Given the extent of the disease, surgical management was deemed necessary. The patient underwent scalpel excision and contouring of the affected nasal tissue to restore nasal structure and aesthetics. Postoperative recovery over 4-6 weeks was uneventful, yielding remarkable results. The nasal contour was significantly improved, with no cosmetic concerns or complications reported by the patient. Long term follow-up showed nil signs of recurrence.
Discussion: This case highlights the unique surgical approach used for the management of rhinophyma, focusing on achieving cosmetic restoration with scalpel excision only in a resource-limited regional setting. It highlights the importance of clinical expertise, creativity, and problem-solving in overcoming equipment constraints. This case serves as a reminder that optimal results can be achieved with the resources at hand despite limitations in regional centers when a patient-centered approach is prioritised.
