Background: Dental sepsis is a common and potentially preventable cause of hospital presentation, particularly in rural and remote regions. These regions face ongoing challenges in access to community dental services, which may contribute to increased acute presentations requiring hospital-based management. Methods: A retrospective review of operative and admission data was conducted at a tertiary referral hospital in Far North Queensland. Cases requiring operative intervention by the Maxillofacial Surgery team for dental sepsis were identified over an 8-year period and examined alongside known reductions in community dentistry availability, such as public dental clinic closures on weekends, holiday periods and during the COVID-19 pandemic. Temporal trends in presentation rates were analysed. Results: Periods of reduced community dental service availability were associated with an increase in hospital presentations for dental sepsis requiring operative management. This was particularly marked during 2020, when dental sepsis presentations nearly doubled that of other years (92, compared to a mean of 45.7 for non-2020 years). Peaks in operative workload occurred on Fridays and Mondays relative to any other days of the week, perhaps reflecting patient perception of limited access to community dentistry alongside reluctance for hospital admission over weekends. Patients frequently presented with advanced infection necessitating inpatient admission, general anaesthesia, and multidisciplinary care. Conclusion: Reduced access to community dental services is associated with increased hospital presentations and operative burden from dental sepsis. These findings highlight the critical role of accessible, continuous community dental care in preventing severe dental infections and reducing avoidable hospital admissions. Strengthening community dental service capacity may improve patient outcomes and reduce pressure on hospital surgical services in rural and regional settings.
