Background
Burns are a common injury. In the Burn Registry of Australia and New Zealand report from 2023/24 captured 2802 cases of burn injuries across 17 burn centres(1). Burns are most associated with thermal injuries, but chemical burns are a diverse group of important injuries. Despite being seen as harmless, natural remedies and plant-based therapies still carry risks with use, especially when administered without appropriate caution.
Case Report:
A 55-year-old man was looking for homeopathic remedies on YouTube to treat his osteoarthritis. He found a recipe and significantly altered quantities and minced three heads of garlic (allium sativum) and combined this with a small amount of holy water. He applied the poultice with gladwrap to his popliteal fossa and posterosuperior calf and fell asleep leaving the mixture on for approximately eight hours. This resulted in a 2% TBSA full thickness burn which required treatment with debridement and split thickness skin graft.
Discussion:
This case report includes a literature review of previous garlic burns. There are various potential benefits of garlic and therapeutic use has been a practice since 3000BC. While it is rare for serious injury, the caustic nature of allicin in raw garlic has been identified in fewer than 50 case reports. While the lower limb was the most common location for garlic burns, our case is atypical due to the depth of the burn likely owing to the duration of application and occlusive dressing.
Conclusion:
This report highlights the need for the public to be aware of the risks associated with natural remedies. Health practitioners should be aware of the possible complications when counselling patients on alternative therapies and managing their complications.
