Ex-Situ Dual Hypothermic Oxygenated Machine Perfusion in Full-Left-Full-Right Split Liver Transplantation for Two Adult Recipients

The scarcity of donor livers remains a major challenge in transplantation. Full-left-full-right (FLFR) split liver transplantation (SLT) addressed donor shortages by enabling transplantation of two grafts into small adult recipients, limited however by prolonged cold ischemia time (CIT) and ischemia-reperfusion injury (IRI). We report a case of ex-situ FLFR SLT utilizing dual hypothermic oxygenated machine perfusion (DHOPE) to mitigate IRI and optimize graft viability. Liver procured from a brain-dead donor was split under continuous DHOPE and transplanted into two adult recipients. At one year follow-up, both recipients demonstrated stable graft function. The use of DHOPE facilitated effective graft preservation and reduced CIT, with no complications exceeding Clavien-Dindo Grade IIIb. This is the first reported ex-situ FLFR SLT utilizing DHOPE for two adults, supporting its feasibility in reducing CIT and improving post-transplant outcomes. DHOPE may serve as an important adjunct in expanding donor pool.
