Consideration of Complete Endovascular Management of Isolated Common Iliac Artery Mycotic Aneurysms

Open surgical resection, extensive debridement, and in-situ revascularization are traditional treatments for mycotic aneurysms but carry high perioperative morbidity and mortality. Endovascular aneurysm repair (EVAR) is a less invasive alternative, yet placing an endoprosthesis in infected fields risks persistent or relapsing infections. To address this, we developed a hybrid technique for excluding common iliac artery mycotic aneurysm (CIAMA) while minimizing surgical risks and infection exposure. Two patients diagnosed with saccular CIAMA, meeting MAGIC criteria, underwent this technique. Symptoms included fever, abdominal pain, microbiology and imaging findings consistent with infection. Post-operative inflammatory markers normalized by day 5, and both were discharged by day 8. Surveillance imaging at 3 days and 4 months confirmed successful CIAMA exclusion, no endoleaks, and infection resolution. Follow-ups at 6 and 12 months showed stable outcomes. Technique Bilateral open common femoral artery exposure is first performed. Internal (IIA) and external iliac artery (EIA) distal to the CIMA is embolized and excluded. Appropriately sized aorto-uni-iliac stent graft was deployed infrarenally across the aortic bifurcation away from the CIAMA. Crossover femoral bypass is then completed to reestablish flow continuity. This hybrid approach presents a viable alternative for patients unsuitable for open surgery. However, it requires favorable vessel anatomy, and long-term data on infection risk are limited. Pre-operative CTA is essential for case selection. Conclusion Aorto-uni-iliac EVAR with femoral bypass effectively excludes CIAMA, avoiding direct prosthesis placement in infected fields and reducing risks of open surgery or persistent infection.
