Title: Comparison of Bleeding Complications Between Bipolar and Monopolar Transurethral Resection of the Prostate (TURP): A Regional Hospital Experience Background: Transurethral resection of the prostate (TURP) remains a standard surgical intervention for benign prostatic hyperplasia. Bipolar TURP has been proposed to reduce bleeding complications compared to the traditional monopolar technique. This study evaluates postoperative bleeding outcomes between bipolar and monopolar TURP in the Taranaki region. Methods: A retrospective audit review was conducted including all public patients who underwent TURP from April 2024 to July 2025 in Taranaki where bipolar TURP became available in April 2024. A total of 111 patients were included: 87 underwent monopolar TURP and 24 underwent bipolar TURP. Primary outcomes were return to theatre for bleeding and clot retention requiring admission. Results: 2 patients (2.3%) in the monopolar group required return to theatre for bladder washout, compared to none in the bipolar group. Clot retention requiring admission occurred in 6 patients (5.4%), with 5 cases in the monopolar group (5.74%) and 1 in the bipolar group (4.2%). Conclusion: Preliminary data suggest that bipolar TURP may be associated with a lower incidence of bleeding-related complications compared to monopolar TURP. Further studies with larger bipolar cohorts are warranted to validate these findings.
