Background: Basal cell carcinoma (BCC) is the most common skin cancer, typically confined to the epidermis. Bony invasion, particularly of the clavicle, is exceedingly rare. The clavicle plays a critical role in shoulder stability and upper limb mobility, making its resection complex with potential for significant functional impairment. Multidisciplinary management is essential to ensure oncological clearance while preserving function and aesthetics.
Case Report: A 75 year old male presented with a palpable mass in the clavicular region, prompting further investigation.
Imaging and biopsy confirmed BCC invading into the clavicle. The patient underwent tumor excision by general surgery and two-thirds clavicle resection by orthopaedics. The tumour was resected with clear margins. Reconstruction was done by plastic surgery using a latissimus muscle flap and split thickness skin graft.
Discussion: This case highlights the importance of a collaborative approach in managing long bone invasion by BCC. Surgical resection and reconstruction were successfully coordinated across specialties, achieving oncological clearance, good cosmetic outcome while preserving majority of shoulder function. Postoperative allied health input ensured comprehensive recovery by emphasising the role of diet, rehabilitation, and home support. This case emphasises the value of a multidisciplinary team in patient care.
