Breast implant exposure with mesh; A case report

Purpose Breast cancer treatment is a difficult journey for patients with multimodal treatment taking up to a year or more to complete. Reconstruction is complex with multiple options, however one must ensure to cater to the individual needs of the patient. Implant based reconstruction with mesh in the setting of prior chemo-radiation is not without risk, a delayed approach may help but appropriate patient consultation is essential. Consideration of implant plane and incorporating new vascularised tissue should be considered. Methodology: A 52-year-old female presented with exposed bilateral breast implants with mesh on a background of prior chemo-radiation and bilateral mastectomy treatment. She had failed multiple attempts of antibiotic conservative treatment as well as attempts to close the skin primarily to prevent further breakdown. The patient was well and desperately wanted to keep her reconstruction. Results: Bilateral removal of breast implants and mesh with washout and closure over a 15 Blake drain was performed. Samples were sent for microbiology culture and sensitivities. The wounds healed nicely, and the patient had further subpectoral expanders placed 4-6 weeks later. Conclusions: The loss of an implant is emotionally tolling for a patient at the end of a long treatment journey. Attempts can be made to salvage an implant but often they require removal to settle the infection. Prior multimodal treatment makes tissue complications more likely and consideration of subpectoral placement or new vascularised tissue flaps should be considered. The role of mesh is still being explored.
