In this report we present a case of a young male, without any significant risk factors, develop a dangerous combination of complications from acute appendicitis. Psoas abscess is a rare complication likely caused by direct spread from the appendix in this case. Similarly, liver abscess can also be a rare complication of appendicitis, likely through haematogenous spread via the portal vein. Both these complications together have not been reported. The patient underwent percutaneous drainage of the liver abscess and total of ten weeks of antibiotic therapy before undergoing colonoscopy and interval appendicectomy.
