Aims:
Joint anaesthesiology and general surgical audit of preoperative fasting times and adherence of our centre to the current WHO and ANZCA guidelines.
Methods:
This was a prospective single centre, observational audit. Data was collected directly from patients, in the form of a structured questionnaire and chart review. Inclusion criteria: Patients >18 years old with capacity to consent who were undergoing elective general surgery requiring fasting.
Results:
Round 1 results:
Data was collected for 25 patients over a two week period. The average fasting duration was 19.4 hours for solid food and 6.8 hours for clear fluids. In the non-bowel preparation group, average fasting duration was 13.4 hours for solid food and 7.2 hours for clear fluids.
Intervention:
Interventions were as follows; optimisation of preoperative surgical fasting advice, increased anaesthesiology advice preadmission and surgical ward based education.
Round 2 results:
6 weeks later, data was collected for 18 patients over a 2-week period. The average fasting duration was 18.2 hours for solid food and 5.3 hours for clear fluids. In the non-bowel preparation group, average fasting duration was 10.5 hours for solid food and 3.8 hours for clear fluids.
Round 3 results:
12 months following the intervention, we completed round 3 of our audit. Data was collected for 19 patients over a 2-week period, however none of this cohort included bowel preparation patients. The average fasting duration was 13.9 hours for solid food and 5.6 hours for clear fluids.
Discussion
Fasting times in our centre for general surgery were initially threefold the recommended guidelines. With our intervention, we saw reduction in fasting times in both bowel preparation and non-bowel preparation groups. However, this improvement was not maintained 12 months later.
