We present a rare case of gallbladder amyloidosis found incidentally in a patient presenting with acute on chronic cholecystitis. The patient had no family history or systemic features of amyloidosis.
A 69-year-old systemically well man was referred to general surgery with a one-week history of epigastric pain and sweating. CT of the abdomen revealed acute cholecystitis with possible cholelithiasis, and a laparoscopic cholecystectomy was performed. However, no gallstones were inside the gallbladder. A post-operative histological examination of the gallbladder wall revealed extensive amyloidosis in the vessel walls.
Since his diagnosis, various specialties have worked together to further investigate this patient for systemic signs of amyloidosis. This case portrays a rare manifestation of amyloidosis in the gallbladder, highlights the importance of further investigating a systemically well patient with subacute acalculous cholecystitis, and demonstrates the importance of the multidisciplinary follow-up that ensues a diagnosis of amyloidosis.
