Background: TikTok dominates contemporary social media as the fastest growing, most popular platform. This medium now encompasses surgical experience which is critical as TikTok’s algorithm can exponentially promote material regardless of prior popularity, engagement or credibility. This can facilitate swift, dynamic surgical education, yet equally, can disseminate an emerging misinformation channel for health information access. Method: A scoping review aimed to review all published TikTok and surgery literature. Pubmed, Embase and MEDLINE searched: TikTok OR Douyin AND Surg.* Results were uploaded into the Covidence systematic review platform. After de-duplication 209 papers were screened via two authors with third author conflict resolution. A knowledge user group (KUG) were consulted for co-design and appraisal. The KUG spanned from medical students to registrars, including individuals who are TikTok users, influencers and TikTok famous. Results: The vast majority of results comprised 84 'TikTok Reviews' analysing TikTok videos relating to surgical experiences and patient education, with metrics of Views, Likes, Shares, or Comments representing engagement. Quality was variably assessed with tools including; DISCERN, GRS, GQS. Content themes included; Educational, Patient experience, Entertainment and Promotional. By speciality, plastics represented most papers, followed by general surgery, ENT and orthopaedics. Conclusion: This scoping review has found a phenomenal breadth of TikTok influence on patient, public and health practitioner perceptions of surgery and surgeons. TikTok can be dangerous with misinformation found to both indirectly and directly cause harm. Conversely this platform offers great benefit for surgeons to capitalise on educational potential.
